چکیده

Radical cystectomy represents nowadays the gold standard in the treatment of localized muscle-invasive bladder cancer and high risk non-muscle-invasive bladder cancer. On the other hand, the rate of morbidity and mortality remains significant despite the advances in perioperative management and postoperative care in recent years [1]. This is even more expressed with the fact that firstly, we talk about a disease affecting eldery people and secondly, the population is aging. Comorbities, and thus the risk of a complication or death, come with age that implies consideration of bladder sparing treatment like radiochemotherapy or partial cystectomy in these patients [2]. EAU guidelines recommend using the Charleson Comorbidity Index as an independent prognostic factor for perioperative mortality [2]. POSSUM and P-POSSUM provide tools for risk adjustment to predict morbidity and mortality which were developed and validated within the general surgical population. A retrospective multicenter study from Japan [3] validated both predictors on 280 patients operated on within a period of 9 years. The authors came to somewhat non-convincing results. POSSUM significantly underestiHow to predict the risk of complication or death after radical cystectomy?

Surgical apgar outcome score: perioperative risk assessment for radical cystectomy.

PURPOSE Currently objective perioperative risk assessment metrics are lacking for radical cystectomy. Using a simple 10-point scale similar to neonatal Apgar assessment we evaluated whether a surgical outcome score calculated immediately after radical cystectomy would predict major complications and mortality. MATERIALS AND METHODS We identified 155 consecutive radical cystectomies performed ...

Incidence and Risk Factors of Acute Kidney Injury after Radical Cystectomy: Importance of Preoperative Serum Uric Acid Level

BACKGROUND Acute kidney injury (AKI) is a common complication after surgery and increases costs, morbidity, and mortality of hospitalized patients. While radical cystectomy associates significantly with an increased risk of serious complications, including AKI, risk factors of AKI after radical cystectomy has not been reported. This study was performed to determine the incidence and independent...

Competing risks of cystectomy – from calculator to decision

Invasive bladder cancer bears the highest risk of death in the short term of all common cancers treated by urologists. Surgery is the only option available to achieve complete cure, but it must be on-time, radical and perfomed by the most experienced surgeons, as complication rates are among the highest of all urological procedures, reaching 50% in high volume centres [1]. Despite lowering peri...

Systematic review and cumulative analysis of perioperative outcomes and complications after robot-assisted radical cystectomy.

CONTEXT Although open radical cystectomy (ORC) is still the standard approach, laparoscopic radical cystectomy (LRC) and robot-assisted radical cystectomy (RARC) have gained popularity. OBJECTIVE To report a systematic literature review and cumulative analysis of perioperative outcomes and complications of RARC in comparison with ORC and LRC. EVIDENCE ACQUISITION Medline, Scopus, and Web of...

Estimation of mortality and morbidity risk of radical cystectomy using POSSUM and the Portsmouth predictor equation

INTRODUCTION The Physiological and Operative Severity Score for the enumeration of Mortality and Morbidity (POSSUM) and the Portsmouth predictor equation (P-POSSUM) are simple scoring systems used to estimate the risk of complications and death postoperatively. We investigated the use of these scores to predict the postoperative risk in patients undergoing radical cystectomy (RC). MATERIAL AN...
